Home Health Team Leader, RN

Westminster-Canterbury on Chesapeake Bay is currently hiring a Home Health Team Leader, RN. Westminster-Canterbury At Home (WCAH) offers personalized and exceptional Home Health Care, Home Care, and Hospice services in private home settings in Virginia Beach and surrounding areas. Schedule: Monday through Friday, 8am to 5 pm (with monthly or bi-monthly on-call responsibilities). What will I do? The Home Health Team Leader, RN is responsible for providing initial and on-going assessment and care planning for home health patients and caregivers. * Supervise and coordinate the implementation of home health orders for all home health clients. * Provide assistance and training to clinicians to promote full understanding of their responsibilities related to documentation and clinical decision-making. * Provide ongoing quality oversight of delivery of home health care. * Supervise, communicate, and collaborate with other members of the home health team. What do I need for this role? * Graduate of an accredited institution of professional nursing. * At least two (2) years of clinical or supervisory experience in home health or related health care is required. * Knowledgeable of applicable local, state, federal regulations. * Holds an unencumbered state license as Registered Nurse. * Current CPR Certification.
