RN Nurse Home Visitor


Job Summary: The RN Nurse Home Visitor (NHV) provides home visits to first-time pregnant women and their babies throughout the pregnancy and until the child turns two years old. The NHV is expected to adhere to the Nurse-Family Partnership (NFP) model of home visitation, which includes: independent critical thinking within the nursing process; assessment of physical, emotional, social, behavioral, and environmental needs of women related to health and life course development, their unborn children, and their families; instruction/education and mentoring in the areas of prenatal and postpartum care, nutrition, parenting and family dynamics, child health care, family planning, and special health care needs; and facilitation of access to resources in the community to improve the outcome of the infant and the future of the client/family. This position will focus on families in Smith County, with some families in Henderson County. Major Responsibilities / Duties / Critical Tasks: ● Supports policies, procedures, guidelines, and standards of NFP and UT Health Science Center at Tyler. ● As requested/required, participates in community coalition in support of NFP. ● As requested/required, promotes public awareness of NFP. ● Develops and maintains community relationships to support client referrals. ● Delivers instruction/education and provides mentoring in the areas of prenatal and postpartum care, nutrition, parenting and family dynamics, child health care, family planning, and special health care needs ● Leverages reflective practice during 1:1 client engagement. ● Conducts independent critical thinking within a community setting ● Completes all required NFP education. ● Attends and participates in NCAST Feeding and Teaching Scales training. ● Develops therapeutic relationships, utilizing concepts of reflection and motivational interviewing with women and their families in a home visiting environment. ● Performs personalized/individualized home visiting in accordance with the need of the client, NFP Model Elements, and NFP Visit-to-Visit guidelines. ● Assesses physical, emotional, social, behavioral, and environmental needs of women, infants, children, and families related to health and life course development, as they relate to the NFP domains following nursing process in implementing NFP model into community nursing practice ● Assists women and their families in establishing goals and outcomes. ● Provides education, support, and referral resources in assisting women and their families in attaining their targeted goals. ● Consults and collaborates with other professionals (healthcare and community) involved in providing services to women and families. ● Formulates nursing diagnosis based on nursing assessment and client goals. ● Evaluates client progress toward stated goals and NFP outcomes and engages client and support systems in adjustments, as needed. ● Manages busy home visiting schedule, ensuring program fidelity and continued trust and engagement of clients and in accordance with client goals and NFP outcomes. ● Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements. ● Meets with NFP Nurse Supervisor weekly for clinical advice, guidance, and supervision. ● Utilizes reflective practice in supervisory sessions. ● Schedules joint home visits with NFP Nurse Supervisor every four months. ● Attends and participates in bi-weekly case conferences, facilitating on a quarterly basis. ● Attends and participates in bi-weekly team meetings, conducting an educational training to peers on a quarterly basis. ● Provides information to support staff for timely and accurate data input to the NFP performance management system. ● Leverages Lean Principles to identify more efficient/effective ways of providing services to clients, reduce waste, and be more effective stewards of grant funds. ● Identify sources of additional support and/or funding to broaden the scope and service to the community ● Participates in quality improvement analysis and planning to identify areas for improvement. ● Maintains confidentiality. ● Documents timely, accurately, and appropriately. ● Performs related duties as assigned or required. ● Understands, supports, and coaches others in the NFP vision, mission, and model. ● Represents the NFP vision, mission, and model in actions and verbally to both internal and external customers and colleagues. ● Assists in creating a positive work environment that promotes productivity, mentoring, teamwork, and cooperation. ● Elicits and considers differing viewpoints when analyzing issues. ● Recognizes and supports accomplishments of team members. ● Maintains clear, effective, open, honest communication with both internal and external customers and colleagues. ● Creates, maintains, and supports a safe environment for open discussion. ● Maintains confidentiality. ● Seeks and responds appropriately to feedback. ● Accurately assesses own learning needs and develops strategies to meet them. ● Utilizes computers for distance learning. ● Stays informed of current health care developments to provide safe, quality nurse home visiting services. ● Establishes and records progress toward annual goals. Accompanying Knowledge, Skills, Abilities and Competencies: ● Ethical, professional, compassionate attributes and share a drive to achieve excellence. ● Excellent written and verbal communication skills, as well as basic computer skills. ● Valid driver's license and insured automobile. ● Ability to work a flexible schedule, including some evenings and weekends. ● Ability to travel for required face-to-face educational sessions and other off-site educational programs requiring maintenance of professional competence and compliance with NFP program requirements. ● Ability to work as part of a team. Required Education / Experience: ● Current RN license or compact RN license to practice in Texas. ● A minimum of 2 years recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing is preferred. ● Current CPR certification.

Pay

Average Texas Staff Position Pay

$30.00/hour

The average salary for a Home Health is 50% lower than the US average of $45.
