Surgical Nurse Navigator - Ortho


Responsibilities Key Responsibilities: * Patient Navigation: * Act as the primary point of contact for VBC patients undergoing orthopedic surgical procedures at the Duly Westmont Ambulatory Surgery Center (ASC). * Gather pertinent patient information, including barriers to care and social determinants of health, to provide personalized support. * Guide patients through preoperative risk assessments, surgical preparations, and post-operative care, ensuring a smooth surgical journey. * Assist surgeons with additional patient communication needs and facilitate necessary rescheduling. * Coordination of Care: * Collaborate with primary care physicians, specialists, surgeons, and the ASC to ensure seamless coordination of patient care. * Facilitate communication among all stakeholders involved in the surgical process, ensuring clarity and continuity of care. * Work closely with care coordination teams at hospitals and other care centers to support transitions of care as needed. * Quality Improvement: * Participate in quality improvement initiatives aimed at enhancing the patient experience and optimizing surgical outcomes. * Provide valuable feedback and suggestions for process improvement to drive continuous improvement. * Education and Counseling: * Educate patients and their families about surgical procedures, potential risks, complications, and postoperative care requirements, empowering them to make informed decisions. * Technology: * Manage the patient engagement platform to interface with patients effectively. * Utilize Electronic Medical Records (EMR) system and scheduling software for seamless coordination of care. * Performance Management: * Monitor relevant metrics and report outcomes in appropriate venues for visibility and accountability. Qualifications Qualifications: * Baccalaureate of Science in Nursing from an accredited program by the Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or Commission for Nursing Education Accreditation (CNEA). * Three years of nursing experience, including a minimum of two years of orthopedic nursing experience. * Current, unencumbered RN license from the Illinois Department of Financial and Professional Regulation.
