Nurse Practitioner, Hospice

Responsibilities The Nurse Practitioner, Hospice is directly responsible for patient care with the collaborating physician. The Nurse Practitioner, Hospice provides a full range of health care services including planning, coordinating and directing optimal patient care and outcomes to assure quality patient care to the patient while meeting regulatory requirements. Essential Functions of the Nurse Practitioner: * The Nurse Practitioner may order the following in order to evaluate, diagnose, and treat the adult and/or geriatric patient: medications (as allowed by regulation and licensure), diagnostic tests, referrals and consultations, measurements, monitoring, procedures, and treatments (as permitted by regulation and licensure.) * Interprets data from the history, physical exam, and diagnostic tests to identify actual or potential patient problems and needs. * Performs health histories. Sources may include, but are not limited to: adult, geriatric patients and/or families, and records requisitioned from Grand View Hospital, other health care providers, or other facilities. * Makes initial comprehensive assessment visit and develops and confirms HPOC and regularly reevaluates patient's nursing needs. * Provides formal and informal education to other health care providers as necessary and/or requested. Schedule: Per Diem shifts available during the week, Monday- Friday for face-to-face evaluations.
