Nurse Practitioner - Phoebe Sumter Primary Care - Ellaville

JOB SUMMARY * Responsible for patient care management, counseling, and the education of patients and families in rural settings. Works in accordance with the Georgia Farmworkers Grant to independently deliver healthcare to migrant and rural agriculture-based communities. Healthcare includes the creation of interventions and preventative planning, development and dissemination of culturally and linguistically relevant educational materials, monitoring communities for health outcomes, and case management. Qualifications EDUCATION REQUIREMENTS * Master's Degree in Nursing (Required) EXPERIENCE REQUIREMENTS * 4+ years experience in supervision and independent decision making in major nursing areas (Preferred) CERTIFICATIONS AND LICENSURES * Required Certifications/Licensures: Nurse Practitioners Certification (NP-C), Basic Life Support (BLS), Department Specific Certifications may be required. * Must possess and maintain a valid Georgia Driver's License and meet auto insurability requirements as approved by Phoebe Putney Health System's insurance company.
