Bilingual Nurse Home Visitor - Part time - $2500 Sign-On Bonus and Housing May Be Available!


Hiring Range: $35-$45 * This is a part-time (24 hours per week) hourly position. This is a great position for new graduates who would like to further their training and certifications or for seasoned nurses who are tired of the crazy schedule and stress that can come along with many nursing positions. Our nurses can develop and foster long-term, therapeutic relationships with clients while having autonomy and full control over their daily schedule. Some remote work is possible. The Nurse-Family Partnership (NFP) - Bilingual Nurse Home Visitor is responsible for providing or assuring comprehensive nursing services to women and their families eligible for the program. The Nurse Home Visitor must maintain the highest standards in clinical nursing practice and adhere to the NFP model and to policies, procedures, guidelines and standards of the NFP program and of Eagle County Public Health and Environment. Bilingual (Spanish & English) fluency is required. $2500 Sign-on Bonus! * Housing may be available! Schedule: The schedule will be flexible and can be based on the applicant's preferred schedule. 24 hours per week, generally between Mon-Fri. Some remote work may be available, however, the position is expected to travel to clients' homes throughout Eagle County and will require to work in the Eagle office weekly. * Applicants are encouraged to apply by Monday, January 29th, however, this position will remain posted until filled. Adheres to the nursing process and the NFP model of home visitation. Carries a caseload of 15-27 clients, depending on FTE. * Develops therapeutic relationships and utilizes concepts of reflection and motivational interviewing with women and their families in a home visiting environment. * Follows the nursing process in implementing NFP into nursing practice. * Performs home visiting in accordance with the NFP model Elements and NFP Visit-to-Visit guidelines. Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains. * Provides education, support and referral resources in assisting women and their families in attaining their targeted goals. Plans home visits in accordance with client goals and NFP outcomes. * Consults and collaborates with other professionals involved in providing services to women and families. Works collaboratively with parents to arrange any needed medical, dental, or nutritional examinations and immunizations. * Completes all required NFP education. Actively engages in professional development to meet all NFP Nurse Home Visitor competency requirements. * Meets with NFP Supervisor for weekly reflective supervision. Attends and participates in weekly team meetings and case conferences. Schedules joint home visits with NFP Supervisor every 4 months. * Follows HIPAA regulations and maintains confidentiality at all times. * Documents appropriately, accurately, and timely. Provides information to support staff for timely and accurate data input to the NFP performance management system. * Promotes public awareness of the NFP program. Develops and maintains community relationships to support client referrals. * Uses a health equity and trauma-informed lens in all work. Acts as a public health role model to exemplify the principles of social justice, increase cultural competence, and work to close gaps in health outcomes that impact populations disproportionately. * Participates in quality improvement efforts. * Observes safety and security procedures, uses equipment and materials safely and reports potentially unsafe conditions. * Serves the community by demonstrating Eagle County Government mission and core values and following relevant policies and procedures. * Other duties as assigned, including support of public health measures benefiting the community at large. Emergency Support Functions May serve in an EOC Operations Center support role during an emergency. Works various shifts to support the EOC and their designated Emergency Support Function (ESF8) during an emergency. Participates in training and preparedness activities as directed by the Emergency Manager. Qualifications Education, Experience, Licensure, Certification, or Registration: * Bachelor's Degree in Nursing from an accredited college or university OR Associate's Degree in Nursing with the ability to pursue Bachelor's Degree in Nursing upon hire is required. * Two years of recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred OR any combination of education, training and nursing experience to meet the needs of the position. * Current Registered Nurse licensure to practice in the State of Colorado is required. Nurses with compact licensure must meet all licensure requirements set forth by the * Colorado Board of Nursing within ninety (90) days of employment. * Current CPR certification is required. * Valid driver's license and satisfactory driving record is required. Knowledge, Skills and Abilities: * Ability to read, write, speak and understand English and Spanish is required. * Ability to express ideas and information clearly and concisely in a manner appropriate to the audience both verbally and in writing. * Ability to provide services to assigned caseload and complete paperwork in a timely manner. * Ability to stay current with basic computer skills. * Demonstrated skill in working together with others through idea-sharing and collaboration. * Ability to perform basic physical and mental health assessments. * Ability to read and interpret a variety of complex rules, regulations, and other materials. * Ability to prepare and maintain accurate and neat records. * Self-motivation and ability to work with minimal supervision. * Modifies own behavior and work activities in response to new information, changing conditions, or unexpected obstacles. * Ability to exercise sound reasoning and analytical thinking to understand and solve workplace problems. * Ability to operate a motor vehicle in various weather conditions. * Ability to occasionally travel in-state for training that may require an overnight stay.
