Integral to our Team Based Care practice, the Anticoagulation Nurse will be responsible for providing warm, comforting care, for patients who have anticoagulation needs. In this position, you will work in conjunction with the Anticoagulation Nurse Supervisor, Pharmacy Manager, clinical pharmacists, anticoagulation nurses and other healthcare professionals to provide excellence in patient-centered healthcare. How You Can Make A Difference * Provide whole-person care to patients enrolled in Anticoagulation clinic programs per clinic guidelines/workflows via portal messages, telephone calls and in-office visits * Perform POC INR tests, interpret INR results, and make medication adjustments according to dosing guidelines * Collaborate with practice pharmacists, Providers and Staff to maintain open communication regarding Anticoagulation clinic patient needs * Maintain current knowledge of anticoagulation practices and continue to grow * Communicate patient care needs to external and internal members of the patient's healthcare team in person, via telephone or electronic health record documentation * Actively participate as a member of the health care team to promote positive health outcomes for our patients and support our healthcare partners. Experience to Drive Change * Registered Nurse required * 3+ years as a Registered Nurse preferred * Experience as an Anticoagulation Nurse preferred * Demonstrated success partnering with patients, physicians, and care teams to support patient care preferred * This role will be based out of the Northern Colorado Timberline or South clinics with days split between these 2 clinics and work from home, depending on Anticoagulation Clinic needs. For Colorado Residents only: The base compensation range for this role is $33.61 - $51.15/hour.
