<h3>Assignment Overview</h3><ul><li>Shift: Days, 5x8hrs</li><li>Hours: 40 hrs/wk</li><li>Start Date: Mar 12, 2026</li><li>Length: 13 weeks</li><li>Openings: 1</li></ul><h3>Description</h3><p>American Traveler is seeking a Physical Therapist for a home health assignment, requiring at least 1 year of experience and active licensure.</p><h3>Details</h3><ul><li>Home health setting with majority adult visits and occasional pediatric patients</li><li>Typical caseload is 5-6 visits per day, or 3 visits plus admissions on days with evaluations</li><li>Monday through Friday, 8:00am–5:00pm schedule with no weekends or holidays</li><li>May be asked to provide occasional safety evaluations or teaching visits for hospice patients</li><li>Utilizes Home Care Home Base EMR system</li><li>Must provide own transportation and maintain current driver’s license and auto insurance</li></ul><h3>Requirements</h3><ul><li>Active ND PT license or compact state licensure required; license must be active at time of consideration (license required if applicable)</li><li>Minimum 1 year of PT experience preferred, with home health and OASIS experience strongly desired</li><li>Current BLS certification required</li><li>Must have a valid driver's license and auto insurance</li><li>Experience with Home Care Home Base EMR preferred</li><li>Two professional references required: one from a supervisor within last 12 months, and one from a peer or supervisor within last 3 years</li></ul><h3>Additional Information</h3><ul><li>Daily responsibilities include performing start of care assessments, recertifications, resumptions of care, and regular therapy visits</li><li>Support resources available from sister agency therapists, clinical coordinator, and director</li><li>Orientation period of 1-2 weeks with preceptor support</li><li>Scrub colors accepted: black, navy blue, or grey</li><li>First-time travelers are accepted for this role</li><li>Must not have been employed (FT, PT, PRN) by any CommonSpirit-affiliated facility in the past 12 months</li><li>Candidates must live at least 50 miles from the facility</li><li>RTO must be declared up front; no additional time off approved after submission</li></ul>
