Incredibly rural CAH ER. Only 3 nurses in building overnights, including you. As ED RN/House Supervisor, you are responsible for registering, triaging, calling provider to come in (sometimes 30 min away) while taking orders over the phone. You then place orders into computer while calling in lab and radiology techs as applicable. If the patient is too critical for the facility, you’re also responsible for arranging EMS or flight transfer to usually Omaha. If there’s patients on the floor and the other nurses are not able (or willing) to come help, this will be ALL you. However, you are still responsible for helping if they need meds from Pharmacy or a pt crashes on the MS unit. When Provider arrives to see pt, you have to put all orders in as verbal. They do NOT place electronic orders, you do. If a pt is admitted, they write paper orders. Nursing is then responsible for placing the orders into patients inpatient EMR, on. program called Thrive. Plus verifying/noting all meds and orders, faxing orders to an offsite Pharmacist; as you’re house supervisor. I think it’s only fair to let others know this is a place you have to have experience to go to & know how to & be prepared to work in a CAH.
