<h3>Assignment Overview</h3><ul><li>Shift: Days, 5x8hrs</li><li>Hours: 40 hrs/wk</li><li>Start Date: Apr 27, 2026</li><li>Length: 13 weeks</li><li>Openings: 1</li></ul><h3>Description</h3>American Traveler is seeking a Physical Therapist with a minimum of 3 years of experience for a 13-week contract in a long-term care/skilled nursing facility setting.<h3>Details</h3><ul><li>Day shift schedule</li><li>Work setting is a long-term care/skilled nursing facility</li><li>Responsibilities span evaluation, treatment planning, and documentation across a resident population</li><li>Electronic Medical Record used for all documentation</li></ul><h3>Requirements</h3><ul><li>Minimum 3 years of physical therapy experience required</li><li>Bachelor's degree required; Master's or Doctorate preferred</li><li>Active PT license in the applicable state required; license required if applicable</li><li>BLS/BCLS certification required</li><li>Prior experience in a long-term care or skilled nursing facility preferred</li><li>Proficiency in Microsoft Office required</li><li>Strong verbal, written, and interpersonal communication skills required</li><li>Ability to lift up to 50 lbs independently; lifts over 50 lbs require a two-person assist</li><li>Must be able to sit, stand, walk, bend, stoop, twist, and reach as part of daily duties</li><li>DOB and SSN may be required for consideration</li></ul><h3>Additional Information</h3><ul><li>Responsibilities include evaluating residents, establishing individualized plans of care with short- and long-term goals, and progressing treatment toward functional outcomes</li><li>Collaborates with an interdisciplinary team including physicians, nursing staff, and other therapy professionals, including PT Assistants</li><li>Provides education to residents and caregivers and documents teaching outcomes</li><li>Participates in team meetings and consults with referring physicians on care plans and discharge needs</li><li>Assists with new program development and provides in-service education to nursing and other departments</li><li>Provides mentorship to new staff during orientation</li><li>Responsible for maintaining department cleanliness and adhering to infection control procedures</li><li>Must maintain all required licenses, certifications, and annual compliance requirements</li></ul>
