      <p>American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of recent experience, OASIS and HCHB proficiency, and flexibility to cover a wide territory.</p>
      
            <h2>Responsibilities</h2>
            <ul><li>Home Health setting serving a largely geriatric patient population</li><li>Daily caseload averages 5-7 patients, with a minimum productivity requirement of 6 visits per day or 30 points weekly</li><li>Covers up to a 60-mile radius from the primary hub or office</li><li>Day shift schedule, 5x8 hours, from 8:00 to 17:30</li><li>On-call coverage required every 3rd to 4th weekend</li><li>Productivity expectations and territory may vary, flexibility is required</li><li>Uses Homecare Homebase (HCHB) EMR system</li><li>First-time travelers are accepted</li><li>Locals are accepted at the travel rate</li><li>Physical therapists are expected to be flexible in covering the assigned territory</li><li>On-call coverage is part of the regular rotation, typically every 3rd to 4th weekend</li><li>Scrubs of any color are required; jeans are not permitted</li><li>An iPad is provided for clinical documentation; cell phone is not provided</li><li>Mileage is reimbursed at the current IRS standard rate</li><li>Requested time off must be submitted at the time of application</li><li>Former employees must have a minimum 6-month separation and HR approval to return</li><li>Travel pairs are accepted but each must cover their own territory with separate transportation</li></ul>

            <ul><li>Home health setting serving adult patients in the region</li><li>Typical caseload of 4-5 adult patients per day, up to 6 visits daily</li><li>1:1 patient care model with productivity based on 27-30 points per week</li><li>Shift is Monday through Friday, 8 hour days (8:00 am – 4:30 pm), with no weekends required</li><li>No on-call requirements</li><li>System groups clients to minimize driving; occasional maximum territory radius may be up to 100 miles</li><li>Mileage is reimbursed at IRS standard rates</li><li>Charting is done in Home Care Home Base; experience required</li><li>Free parking provided</li><li>Any color scrubs accepted</li><li>Primary responsibility is providing skilled therapy visits and completing documentation thoroughly and on time</li><li>Travelers are expected to return documentation tools at end of assignment</li><li>Territories and client lists are managed to support efficient scheduling</li><li>First-time travelers are accepted for this assignment</li><li>Travel pairs accepted, each assigned to separate territories and must have own transportation</li><li>All time off requests must be submitted with application</li><li>Former perm staff must be separated for at least 6 months before returning as a traveler and require HR approval</li></ul>
