St. Catherine’s Rehabilitation Hospital is a 22-bed state-of-the-art medical facility designed for patients who can benefit from intensive rehabilitation in an interdisciplinary hospital setting.

An injury or illness may lead to disabilities however, through specialized medical rehabilitation, many patients regain function and learn or improve skills that can compensate for lost abilities.

The St. Catherine’s Rehabilitation Team works with one important goal in mind; to design a program individualized for each patient that helps them to achieve their maximum level of functional independence and return to the community as quickly as possible.

Our specialized rehabilitation programs are designed to meet the needs of individuals who have a broad range of acute or chronic illness that cause temporary or permanent disability. Our programs combine medical care with rehabilitation services to help re-establish the functional, physical, cognitive and behavioral skills required to live as independently as possible. Physical, occupational, speech and respiratory therapies will be provided, as well as physician supervision and round-the-clock nursing care.

The Wound Care Nurse is responsible for developing, coordinating, monitoring and managing the facility Wound Care Program to ensure the delivery of care meets the standard of care.

Essential Functions

* Initiates a nursing evaluation of patient and/or resident's skin condition upon admission. Evaluates and interprets reports communicating findings with the assigned physician. Implements an appropriate treatment plan for all newly assessed wounds to ensure that standards of care are being met.
* Documents status of wound healing and treatment provided on a weekly basis.
* Reviews skin risk evaluation for all newly admitted patient and/or resident's with significant change.
* Prepares and submits wound report as needed.
* Coordinates wound care rounds in the facility and participates in the interdisciplinary process of the plan of care, treatment and services for each patient and/or resident reviewed.
* Contacts physicians for treatment order based on recommendations from interdisciplinary team rounds.
* Performs wound care treatments as ordered.
* Answers call lights within appropriate established timeframe.

Other Duties

* Provides consultation to facility staff, physicians and other professionals on pressure ulcer prevention and treatment according to defined protocols.
* Provides in-services and training relating to skin and wound care as needed.

The Manager, Nurse is a registered nurse professional who is responsible for overseeing the clinical aspect of nursing care on the assigned area to ensure that the delivery of care meets the department's professional as well as regulatory standards. The Nurse Manager has 24/7 responsibility of his/her specific unit.

Essential Functions

* Plans, organizes, directs, controls and evaluates delivery of patient and/or resident care and services.
* Responsible to complete appropriate daily nursing assignments to ensure patient and/or resident care is met.
* Supervises staff (licensed and non-licensed) in the assessment, planning, implementation and evaluation of care.
* Completes employee evaluations on a timely basis. Accurately and objectively assesses performance.
* Keeps members of the interdisciplinary team informed of changes in resident status.
* Ensures compliance of standard of nursing care on assigned shift by assisting licensed staff in planning, controlling and evaluating nursing care during the shift.
