            <h2>Responsibilities</h2>
            <ul><li>Home health setting serving patients in Sonoma, CA and surrounding areas</li><li>Visits are one-on-one with up to 6 patients per day, maintaining a standard of 30 productivity points</li><li>Day shift with 8-hour shifts from 8:00 to 16:30, five days a week</li><li>One weekend shift required per contract</li><li>No floating to other units required</li><li>No on-call requirement during this assignment</li><li>EMR used is HomeCare HomeBase (HCHB); proficiency required</li><li>OASIS documentation required for Start of Care visits</li><li>Mileage reimbursement offered at IRS standard rates</li><li>No standard attire or scrub color required</li><li>Main responsibilities include providing skilled physical therapy services to home health patients and completing comprehensive OASIS assessments</li><li>System groups clients to minimize drive time, but some travel may extend beyond a 50-mile radius</li><li>Documentation and timely communication are essential responsibilities</li><li>Free parking is available for staff</li><li>Travel pairs are accepted but each traveler must have their own transportation and will be assigned to separate territories</li><li>Former permanent employees must have a minimum 6-month separation and receive HR approval before returning as a traveler</li><li>RTO (Requested Time Off) must be submitted at the time of application</li><li>Assignments are open to local candidates within a 50-mile radius</li></ul>

            <ul><li>Home health setting with a focus on geriatric patients</li><li>Typical caseload of 5-7 patients per day, to be confirmed during interview</li><li>Day shift schedule, Monday through Friday from 8:00 am to 4:30 pm</li><li>Service area covers up to a 60-mile radius from the office</li><li>No call required</li><li>Charting completed using HCHB (Homecare Homebase) EMR; experience required</li><li>iPad provided for documentation</li><li>Mileage reimbursed at the IRS standard rate</li><li>Dress code requires scrubs of any color; jeans are not permitted</li><li>All charting must be completed by 9:00 am the following morning</li><li>Job duties include delivering OT services in home environments and meeting weekly productivity expectations</li><li>Applicants must not be first-time travelers</li><li>Local and travel candidates are both accepted at the travel rate</li><li>Requested time off must be submitted with application</li><li>Travel pairs are accepted but each therapist will be assigned a separate territory and must have their own transportation</li><li>Former perm employees must be separated for at least 6 months and require HR approval to return as travelers</li></ul>
