<h3>Responsibilities</h3><ul><li>Execute physician prescribed plans of care </li><li>Complete compliant documentation of the care provided in the system of record</li><li>Manage and administer prescribed medication, treatment, and therapies</li><li>Conduct patient assessments, and assist coordination of care</li><li>Perform various activities associated with daily living</li><li>Monitor vitals</li><li>Assess skin integrity and administer wound care</li><li>Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression</li><li>Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients</li><li>Provide education and training to the patient, their family, and/or the Home Health Aide</li><li>Work with patients to be done one-on-one in the patients home to provide for a comfortable environment</li><li>Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants</li><li>Perform other personal care services as necessary to meet the patient’s needs</li></ul><h3>Minimum Qualifications</h3><ul><li>Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing</li><li>Current State or Compact License as a Registered Nurse </li><li>TB test and physical examination</li><li>Current CPR certification</li><li>Maintain required insurance</li><li>Good interpersonal skills</li><li>Hand-eye coordination and manual dexterity</li><li>Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients</li><li>Ability to operate under stressful conditions and make quick decisions</li><li>Must have dependable transportation and be willing to commute to the patient’s home</li><li>Ability to provide basic medical management up to medically fragile end of life care</li></ul><h3>Preferred Qualifications</h3><ul><li>1+ years of experience as an RN in a clinical or home health setting.</li><li>Ability to assist with two person assist</li></ul>$50-55/hr for Registered Nurses $1500 engagement bonus!
