This role offers the opportunity to provide compassionate care to geriatric patients with psychiatric needs, ensuring their well-being through comprehensive nursing care and collaboration with a multidisciplinary team.

Responsibilities

• Patient Care
  • Maintain accurate patient reports and medical histories
  • Perform mental and physical assessments
  • Administer medication and treatment to patients
  • Provide assistance to patients with daily activities and observe reactions
  • Conduct therapies in accordance with patient's diagnosis and needs
  • Perform treatment interventions following physician orders such as medication administration and medical treatment orders
• Treatment Planning
  • Complete individualized treatment plans for each patient upon admission and ongoing with the multidisciplinary treatment team
  • Plan, implement, and coordinate nursing care activities for patients
  • Manage activities within the therapeutic milieu including daily groups, education, and 1:1 with individual patients
  • Collaborate with physicians, multidisciplinary team members, and outside health and social agencies to ensure the best possible care for patients
  • Provide efficient discharge plans and obtain accurate histories at admission
• Documentation
  • Complete all required documentation relevant to patients in a timely, efficient manner. This includes but is not limited to:
    • Admission paperwork
    • Assessments (mental and physical)
    • Progress notes
    • Interventions
    • Evaluations
    • Discharge paperwork
    • CQI data
• Quality of Care
  • Assure quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and regulatory standards

Job Requirements

• Education
  • 2-year Associates degree or diploma from a 3-year Nursing program required
  • Bachelor's degree preferred
  • Must possess valid State of New York RN license
  • BLS, ACLS, NRP certification required
• Experience
  • Minimum 2 years of nursing experience required
  • Medical terminology required
• Other Skills Required
  • Excellent mathematical and analytical skills
  • Excellent written, verbal, and interpersonal communication skills
