Responsibilities

• Manage dysphagia and provide aural rehabilitation.
• Develop and implement functional maintenance programs.
• Conduct patient and family education sessions.
• Perform evaluations using tools such as BIMS, Depression Screening, and Oral Peripheral Exam.
• Ensure patient safety and deliver high-quality care in home health settings.
• Complete billing and documentation accurately and timely.
• Participate in in-service training and contribute to interdisciplinary support teams.

Requirements

• Minimum 2 years of experience in speech-language pathology.
• Home health experience required; OASIS training preferred.
• Active Florida or Compact State License.
• Certifications: BLS, CCC-SLP.
• Valid driver's license and personal vehicle for home visits.
• Must be COVID vaccinated or have an exemption form on file.
• Must have flu shot or declination on file; mandatory mask wearing at all times while at work in both cases.
• Proficiency in evaluation and assessment tools, as well as documentation systems such as Epic.
