Description
Join WCS Healthcare Partners as a dedicated Physical Therapist in our Home Health program. We are seeking an experienced professional to provide exceptional care to our patients in the comfort of their homes. This part-time position offers the opportunity to make a meaningful impact in the community, servicing various locations in and around Plainsboro, NJ.
Responsibilities
- Conduct comprehensive assessments and develop individualized care plans for patients.
- Provide physical therapy treatments to patients in their homes, ensuring high-quality care and patient safety.
- Collaborate with other healthcare professionals to optimize patient outcomes.
- Educate patients and their families on therapy techniques and self-care practices.
- Maintain accurate and timely documentation of patient care activities.
Requirements
- Minimum of 2 years of experience in physical therapy, with start of care and revisit experience required.
- Active PA license required to submit.
- Basic Life Support (BLS) certification required.
- Advanced Cardiovascular Life Support (ACLS) and Neonatal Resuscitation Program (NRP) certifications required.
- Ability to service the following zip codes: 19040, 19044, 18929, 18974, 18976, 19002, 18954, 18966, 18901.
