Description

WCS Healthcare is currently seeking an Case Management Nurse for a contract assignment at a Prestigious Hospital in Melrose, MA.

Job Function:

Day Shift.
Assess new patients.
Update and revise patient health care plans as needed.
Educate patients and families about health care plans and options.
Make clinical decisions for routine patient care.
Accurately document and submit medical records.

Job Requirements:

Mus have Inpatient Hospital Case Management experience.
BSN preferred but not required.
Experience in Acute care Setting, plus experience in discharge.
MA Licenses.
BLS.
