Description

**Job Title: Home Health LVN/LPN • Days**

Join our team in Dorchester, MA, as a Home Health LVN/LPN for a 13•week contract, with potential extension. Work 40 hours per week on day shifts.

**Responsibilities:**

• Provide quality home health care to patients.
• Manage a daily caseload of 6 patients.
• Collaborate with healthcare teams for comprehensive care.
• Document accurately, HCHB experience preferred.

**Qualifications:**

• Valid MA LVN License.
• BLS Certification.
• 2+ years in Home Health.
• Strong communication and organization skills.

**Details:**

• No on•call duties.
• Rare floating to other locations.
