Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC

• Facility Type: Home Health
• Shift Name: Days
• Shift Details: Flexible
• Schedule: Flexible, 5x8s or 4x10s
• Mileage Rate: $0.56
• Miles Per Day: 45 miles max (anything over 45 miles gets paid at a higher mileage rate)
• Weekly Productivity Expectation: 30 points per week (6 per day)
• Weekend Rotation: No
• On-Call Requirements: No
• EMR System: Wellsky & Oasis
