Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Michigan Physical Therapist license required to start the assignment.
- BLS (Basic Life Support) certification required from AHA or ARC.
- Home Health experience required. Candidate must be certified or have demonstrated competency working in home health settings.
- Experience with EPIC electronic health record preferred.
- Ability to complete required documentation and travel to patient residences within assigned service area.
- Preferred: prior travel or agency experience and experience managing caseloads in community or home-based care.

- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Monday - Friday, 08:00 - 17:00.
- Holiday rotation: no weekends or holidays.
- Start: 04/06/2026. Assignment length: 12 week.
