Minimum Qualifications

• Associate degree in Physical Therapy Assistant from an accredited educational program.
• Active Illinois PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in skilled nursing facility or long-term care settings preferred.
• Experience providing restorative or maintenance therapy and working with geriatric patient populations preferred.
• Ability to document in electronic medical records preferred.

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Days 08:30–16:30.
• Work Schedule: 5x8 (five days per week).
• Contract Length: 14 weeks.
• Guaranteed Hours: 35 hours per week.
