Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Current Physical Therapy license in the state of Texas required to start the assignment.
- Current CPR certification required.
- Must have a current driver’s license, vehicle insurance, and reliable personal vehicle for travel within the agency service area.
- Home Health and Homecare/Homebound (HCHB) experience required for this position.
- Ability to complete patient evaluations, develop plans of care, and document per state and CMS guidelines.
- Ability to participate in a weekend rotation and be available on observed holidays as needed.
- Preferred: Prior experience supervising PTAs and home health aides, if allowed by state regulation.

- Clinical Duties: Evaluate functional status, develop and implement PT plan of care, perform interventions to improve mobility and function, document progress per agency policy, and provide patient/caregiver education.
- Travel Requirements: Visits patients across the agency service area and may cover nearby agency service areas as needed.
