Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Current Physical Therapy license in Oklahoma required to start the assignment.
- Current CPR certification required.
- Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle for travel throughout the agency service area.
- Experience providing physical therapy in home health settings preferred.
- Ability to complete patient evaluations, develop plan of care, and document per CMS and agency policy within required timeframes.
- Ability to participate in a weekend rotation and be potentially available on observed holidays as needed.
- May supervise PTAs and home health aides when permitted by state regulation and agency policy.
