Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy license in Oklahoma required to start the assignment.
• Current CPR certification required.
• Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle for travel throughout the agency service area.
• Experience providing physical therapy in home health settings preferred.
• Ability to complete patient evaluations, develop plan of care, and document per CMS and agency policy within required timeframes.
• Ability to participate in a weekend rotation and be potentially available on observed holidays as needed.
• May supervise PTAs and home health aides when permitted by state regulation and agency policy.
