Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred. Experience performing skilled evaluations and home exercise program instruction preferred.
• Ability to document in electronic medical records required. Experience with home health documentation systems preferred.
• Valid driver’s license and reliable transportation required for travel between patient homes.
