Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program.
• Active Ohio Physical Therapy license required prior to start of assignment.
• Current CPR certification.
• Valid driver’s license, proof of vehicle insurance, and access to a dependable vehicle for patient visits across the service area.
• Experience providing home health physical therapy preferred.
• Ability to complete patient evaluations, develop plans of care, and document per CMS and agency requirements.
• Schedule flexibility to participate in weekend rotation and potential holiday availability as needed.
