Minimum Requirements

• Master's Degree in Speech-Language Pathology from an accredited educational program
• Completed CFY and current Certificate of Clinical Competence from ASHA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Michigan speech license and ASHA certification
• Driver's license with picture identification
• Background check
• Skills checklist from Agency
• Resume/application
• Certifications (BLS, ACLS)
• Records of nursing related competencies/exams
• Tuberculosis test records/Hepatitis Vaccine record
• Influenza or declination
• Copy of agency's Certificate of Liability Insurance
• Drug Test
• Attestation form
