Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Tennessee Physical Therapy license is required to start the assignment.
• Current CPR certification.
• Valid driver’s license, current vehicle insurance, and access to a dependable vehicle for patient visits across the agency service area.
• Experience providing home health physical therapy services or equivalent outpatient/community-based care preferred.
• Ability to complete patient evaluations, develop plans of care per state guidelines, and submit documentation within required timeframes.
• Willingness to participate in weekend rotation and be available on observed holidays as needed.
• Ability to supervise PTA and home health aides when permitted by state regulation and agency policy preferred.
