Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Tennessee Physical Therapy license is required to start the assignment.
- Current CPR certification.
- Valid driver’s license, current vehicle insurance, and access to a dependable vehicle for patient visits across the agency service area.
- Experience providing home health physical therapy services or equivalent outpatient/community-based care preferred.
- Ability to complete patient evaluations, develop plans of care per state guidelines, and submit documentation within required timeframes.
- Willingness to participate in weekend rotation and be available on observed holidays as needed.
- Ability to supervise PTA and home health aides when permitted by state regulation and agency policy preferred.
