Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Colorado Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in outpatient physical therapy required, including evaluation and development of individualized treatment plans.
• Ability to perform 60-minute evaluations and 30-minute follow-up treatment sessions as part of routine scheduling.
• Documentation competency, including goal writing, functional quality indicator assessment, letters of medical necessity, and charge entry in compliance with state and federal guidelines.
• Experience supervising and delegating to therapy aides or assistants and participating in student clinical education preferred.
• Maintain professional licensure in good standing and complete state-mandated continuing education requirements.

• Shift Pattern: 5x8 schedule.
• Duration: 13-week assignment; client indicates preference for a 6-month contract.
• Caseload: Evaluations at 60 minutes with follow-up visits at 30 minutes.
