Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Arizona license required for submission or must have already applied and able to provide an ETA for license approval
• Experience in skilled nursing environments preferred
