Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapist licensure in Illinois required to start the assignment.
• Current CPR certification required.
• Must have a valid driver’s license, vehicle insurance, and reliable vehicle for patient home visits.
• Experience providing home health physical therapy preferred. This includes evaluating functional status, developing and implementing home-based plans of care, and documenting per CMS requirements.
• Ability to complete evaluations and develop PT plans of care within state-specific guidelines and to report outcomes to the nurse supervisor within 24 hours of evaluation.
• Schedule flexibility required. Must be able to participate in weekend rotation and potential observed holiday availability as needed.
• May be required to supervise Home Health Aides and PTAs in accordance with state regulation and agency policy. Previous supervisory experience preferred.
