Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona state license or ability to obtain state licensure prior to assignment start.
• BLS certification from AHA or ARC may be required.
• Experience in outpatient orthopedic physical therapy required.
• Competence with comprehensive patient evaluation, goal writing, functional treatment planning, and documentation in compliance with state and federal guidelines.
• Experience with documentation of letters of medical necessity and charge entry preferred.
• Ability to supervise and delegate to support staff and to participate in student experiential programs preferred.
• Adherence to applicable State Practice Act, maintenance of professional licensure, and completion of required continuing education.
