Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active Arizona Physical Therapy license required to start the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience in outpatient orthopedic physical therapy required.
• Demonstrated competency in comprehensive evaluation, goal writing, and development of individualized treatment plans.
• Experience with documentation, letters of medical necessity, charge entry, and use of electronic medical records in compliance with state and federal guidelines.
• Ability to supervise and delegate to PTA/aides and to participate in student clinical education as needed.
• Commitment to patient and employee safety, including safe patient handling and infection control practices.
• Maintain professional licensure and complete required continuing education in accordance with state practice act.
