Minimum Requirements

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy license in Ohio required to start the assignment.
• Current CPR certification required.
• Valid driver’s license, vehicle insurance, and reliable personal vehicle for patient visits.
• Experience providing home health physical therapy or equivalent outpatient/short-term rehabilitation experience preferred.
• Ability to complete evaluations and develop plans of care according to state guidelines and CMS documentation requirements.
• Willingness to participate in weekend rotation and potential availability on observed holidays as required by the agency.
• Ability to supervise and coordinate with PTAs and home health aides when allowed by state regulation.
