Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Current Physical Therapy license in Oregon is required to start the assignment.
- Current BLS/CPR certification required.
- Must have a current driver's license, vehicle insurance, and access to a dependable vehicle for patient visits.
- Experience providing home health physical therapy preferred.
- Ability to complete comprehensive evaluations, develop and document PT plans of care, and submit progress notes per agency policy within required timeframes.
- Experience supervising PTAs and home health aides preferred, if allowed by state regulation.
- Schedule flexibility required, including participation in a weekend rotation and potential availability on observed holidays.
