Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy license in Oregon is required to start the assignment.
• Current BLS/CPR certification required.
• Must have a current driver's license, vehicle insurance, and access to a dependable vehicle for patient visits.
• Experience providing home health physical therapy preferred.
• Ability to complete comprehensive evaluations, develop and document PT plans of care, and submit progress notes per agency policy within required timeframes.
• Experience supervising PTAs and home health aides preferred, if allowed by state regulation.
• Schedule flexibility required, including participation in a weekend rotation and potential availability on observed holidays.

Provide evaluations, develop individualized plans of care, implement evidence-based interventions, and document patient progress in accordance with state laws and agency policies.
