Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
- Current Michigan Physical Therapy license required to start the assignment.
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and access to a dependable vehicle.
- Home health experience required.
- Experience with HCHB (Homecare Homebase) EMR required.
- Ability to participate in weekend rotation and potential holiday availability as needed.
- Ability to perform home visits across a broad service area and accept geographically grouped schedules.
- Preferred: Experience supervising PTAs and home health aides per state regulation.
Assignment Details
- Facility type: Home Health.
- Shift name: Other.
- Weekly point expectation: 24 points for a guaranteed 32-hour contract.
- Typical productivity: approximately 6 points per day (points vary by visit type).
- Visit types and points: Start of care/admission 2.5; Add-on eval 1.25; Recertification 1.75; Resumption of care/recertification 2; Subsequent visit 0.9; Agency discharge 1.15; Discipline-only discharge 1.
- Primary coverage area: Wayne County with additional coverage across Oakland, Macomb (Sterling Heights), Livingston (Brookdale Brighton), Genesee (Brookdale Grand Blanc), and Washtenaw counties.
- Clinicians expected to travel across the service area. Scheduler will group visits by geography to limit drive time when possible.

Impacting Patient Care
Provide assessments, develop plans of care, perform interventions to improve patient function and pain, document per CMS and agency standards, coordinate with physicians and interdisciplinary team members, complete timely progress notes and 60-day summaries, and participate in required rotations. Adhere to state scope of practice and agency standards. Maintain licensure and continuing education as required.
