Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy license in New Mexico required to start the assignment. Candidates with multi-state licensure or ability to obtain licensure in NM are preferred.
• Current CPR certification required.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle for home health visits.
• Experience in Home Health settings preferred. Experience evaluating functional status, developing and implementing PT plans of care, and completing CMS-compliant documentation is required.
• Ability to perform patient evaluations, develop treatment plans, and complete progress notes and discharge summaries per agency policy and state regulations.
• Ability to participate in weekend rotation and potential holiday availability as needed.
• May be required to supervise Home Health Aides and PTAs in accordance with state regulation and agency policy. Supervisory experience preferred.
