Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Kentucky Physical Therapist license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience required. Experience providing in-home evaluations, skilled treatment planning, and patient education is preferred.
• Client requires coverage across Todd County and Logan County service areas; candidate must be able to travel and provide care across these counties.
• Reliable transportation and valid driver’s license required for in-home visits.
• Experience with documentation in electronic medical records preferred.
