Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active South Carolina physical therapy license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Home health experience required. Experience with in-home patient care and documentation preferred.
• Familiarity with OASIS assessments, Medicare Conditions of Participation for Home Health, and related regulatory requirements preferred.
• Ability to perform moderate physical activity including frequent walking, standing, bending, lifting up to 50 pounds, and driving for patient visits.
• Experience managing caseloads independently and documenting care accurately in clinical records preferred.

Assignment Details

• Facility Type: Home Health
• Shift Name: Days
• Shift Details: Days 0800-1700
• Service Area: Clarendon, Sumter, Lee, Darlington, and Florence counties (service areas may require travel within these counties).
• Expectation to provide at least 20 in-home patient care units per week.
• Orientation: Agency allows up to two weeks of orientation pay with pay equivalent to 30 units.
• Limited requested time off; requests should not extend more than two consecutive days.
• Floating: May be required to travel within the listed service counties for patient visits.
