Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active Tennessee PTA license or ability to obtain state license prior to start.
• BLS certification from American Heart Association or American Red Cross may be required.
• Experience in skilled nursing facility (SNF) setting required. New graduates accepted if they have SNF clinical experience.
• Experience with point-of-service documentation systems preferred. Facility uses NetHealth and expects productivity targets (approximately 88% productivity).
• Ability to coordinate with a small therapy team; current staffing includes PT plus one more PTA or two PTs.

• Facility type: Skilled Nursing Facility (short-term, long-term, and outpatient care).
• Shift: Days.
• Patient census: 136-bed facility.
• Documentation: NetHealth point-of-service documentation is used.
• Team structure: Collaborative work with onsite PTs and PTAs; caseload coordination expected.
