Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapy license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Demonstrated competency evaluating and treating geriatric populations, including fall risk assessment and restorative therapy preferred.
• Experience with documentation in electronic medical record systems preferred.
