Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Current Physical Therapy license in Delaware or ability to obtain state licensure prior to start.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and reliable transportation to visit patients in the community.
• Experience in home health, skilled nursing, or community-based settings preferred.
• Ability to evaluate functional status, develop and implement a plan of care, document per CMS and agency requirements, and prepare discharge summaries within required timeframes.
• Experience supervising PTAs and home health aides preferred, contingent on state regulations.
• Schedule flexibility to participate in weekend rotations and potential observed holiday availability as needed.

Assignment Details

• Facility Type: Home Health.
• Position Type: Travel, full time.
• Clinical Responsibilities: Patient evaluations, development of PT plan of care, implementation of interventions to improve function and reduce pain, progress documentation per agency policy, communication with physicians and nursing staff, and preparation of discharge summaries at least every 60 days.
• Geographic Coverage: Visits patients across the agency service area and may include nearby agency service areas; candidate must be willing to travel within the region.
• Supervisory Duties: May supervise home health aides and PTAs when permitted by state regulation and agency policy.
