Make a meaningful difference as a travel OT with Jackson Therapy Partners. We are seeking a dedicated and compassionate Occupational Therapist to help patients with injuries, illnesses, or disabilities regain or improve daily living skills while exploring Lancaster, OH.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited educational program.
• Active Occupational Therapy license in Ohio is required to start the assignment.
• Current CPR certification required.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle, or reliable public transportation for home health travel.
• Experience providing occupational therapy in a Home Health setting preferred.
• Experience conducting home safety assessments, recommending adaptive equipment, and documenting per CMS and agency requirements preferred.
• Ability to complete evaluations and develop OT plan of care in accordance with state-specific guidelines and submit outcomes to supervising nurse within 24 hours.
• Willingness to participate in weekend rotation and availability on observed holidays as needed.

• Clinical Responsibilities: Conduct patient assessments, develop and implement OT plans of care, document progress notes per agency policy, provide caregiver education, and prepare written discharge summaries at least every 60 days.
• Collaboration: Coordinate with physicians, nursing supervisors, and interdisciplinary team members. May supervise OTAs and home health aides as permitted by state regulation.
• Travel Requirements: Visits patients throughout the agency service area and may float to other nearby agency locations as needed.
