Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body.
• Current, unencumbered New Hampshire physical therapist license or active license for the state the clinician will practice in prior to start.
• BLS/CPR certification required.
• Experience providing home health physical therapy and conducting home visits. Must have reliable transportation and valid driver’s license.
• Familiarity with electronic documentation and basic computer skills, including email and online training platforms.
• Client-preferred: 2–3 years of physical therapy experience.

Assignment Details

• Schedule format: 5x8.
• Must supervise and evaluate PTAs and home health aides as assigned.
• Must perform patient evaluations, develop and update plans of care, document condition and progress, and provide in-home treatment per physician orders.
• May be required to float to other nearby facilities within the agency network.
