Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Tennessee physical therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in Skilled Nursing Facility (SNF) settings required. New graduates will be considered if they have SNF clinical experience.
• Willingness to complete facility-required acknowledgement form if not vaccinated for COVID-19.
• Ability to meet productivity expectations (client indicates productivity target around 88% with point-of-service documentation).
• Point-of-service documentation experience preferred.

• Facility type: Skilled Nursing Facility.
• Shift name: Days.
• Schedule: Monday through Friday, 8-hour shifts. Flex schedule available and required when needed.
• Setting requires flexibility to accommodate facility needs.
