Minimum Requirements

• Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active New York State Physical Therapy license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience working in a skilled nursing facility preferred.
• Experience performing evaluations and providing skilled PT services to post-acute and long-term care residents preferred.
• Ability to participate in interdisciplinary morning meetings and assist with team-based care and additional clinical tasks as requested by the facility.
• Comfort with documentation in electronic medical records preferred.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Days 08:30–16:30.
• Work Schedule: 5 days per week, 8-hour shifts.
• Assignment Length: 13 weeks.
• Clinician responsibilities include evaluations, treatment, and participation in morning meetings; candidate should be comfortable assisting with additional departmental tasks.
