Minimum Requirements
- Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Florida PT license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Home health experience is required. Experience with HCHB electronic medical record preferred.
- Productivity experience or ability to meet productivity targets. Client productivity requirement: 6 points per day / 30 points per week.
- Ability to maintain weekly mileage at or under roughly 100 miles per week is preferred.
- Ability to collaborate in a small, team-oriented office environment. Orientation will be provided.
- Preferred: experience working with PTAs and OTs in outpatient or home health settings.
Assignment Details
- Facility type: Home Health.
- Shift: Days.
- Schedule options: 5x8 (Monday–Friday) or 4x10s.
- EMR: HCHB.
- Productivity expectation: 6 points per day / 30 points per week.
- Mileage guidance: aim to stay under 100 miles per week.
- Team composition: current staff includes multiple full-time PTs, PTAs, and OT staff; small office, team oriented.
