Minimum Requirements

• Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Florida PT license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience is required. Experience with HCHB electronic medical record preferred.
• Productivity experience or ability to meet productivity targets. Client productivity requirement: 6 points per day / 30 points per week.
• Ability to maintain weekly mileage at or under roughly 100 miles per week is preferred.
• Ability to collaborate in a small, team-oriented office environment. Orientation will be provided.
• Preferred: experience working with PTAs and OTs in outpatient or home health settings.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Schedule options: 5x8 (Monday–Friday) or 4x10s.
• EMR: HCHB.
• Productivity expectation: 6 points per day / 30 points per week.
• Mileage guidance: aim to stay under 100 miles per week.
• Team composition: current staff includes multiple full-time PTs, PTAs, and OT staff; small office, team oriented.
