Minimum Qualifications

• Associate degree in Physical Therapist Assistant or equivalent from an accredited educational program.
• Active Arizona PTA license is required for submission or candidate must have already applied and be able to provide an estimated timeline for license approval prior to the assignment.
• Current BLS certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility or long-term care setting preferred.
• Ability to follow POC developed by supervising Physical Therapist and document care in electronic medical record systems.
• Ability to work a 32-hour work week with availability for a 4x8 days schedule as required by the client.
• Preferred: prior experience with restorative nursing programs and mobility/transfer training for older adults.

Assignment Details

• Facility type: Skilled Nursing Facility.
• Shift name: Days.
• Schedule: 4 x 8 shifts per week (client indicates a 32 hour guarantee).
• Assignment length: 13 weeks.
• Arizona license required for submission or must have applied and be able to provide an ETA for approval.
