Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active Illinois PT license required prior to start of assignment.
• BLS certification from AHA required.
• Minimum 2 years recent Home Health physical therapy experience with adult/geriatric general medical-surgical patients. (Client will consider 1 year if the candidate has recent Home Health experience.)
• Demonstrated ability to assist with ADLs and adaptive equipment and to collaborate effectively with RN, OT, Speech and other team members.
• Valid driver’s license, reliable personal vehicle, and auto insurance required. Candidate must provide own transportation to patient visits.
• Ability to complete orientation and EMR skills check-off (approx. 2 days onsite in Oak Brook, IL).
• Preferred: prior EPIC EMR experience.

Assignment Details

• Facility Type: Outpatient / Home Health coverage model.
• Schedule cycle: Monday through Friday, shift times approximately 8:00–17:00.
• Expect to conduct approximately 5–6 patient visits per day within the designated coverage area.
• Orientation: approximately 2 days onsite in Oak Brook, IL for EMR and skills check-off; subsequent work is remote (no physical office; meetings held virtually).
• Coverage Area: Central Metro E ZIPs including 60630, 60618, 60625, 60646, 60659, 60660, 60657, 60640, 60613, 60614 and surrounding areas.
• Floating: may be required to cover within the specified coverage area.
• Patient types: adult and geriatric general med/surg patients.

Standard Job Description

Provide home health physical therapy to adult and geriatric patients within the designated coverage area. Perform assessments, create individualized treatment plans, implement therapeutic interventions focused on mobility, ADLs and adaptive equipment, and coordinate care with interdisciplinary team members. Complete documentation in EPIC and participate in virtual meetings. Supplies and equipment will be delivered to the clinician's home.

Additional Information

• Attire: solid color scrubs or business casual. No jeans or open-toe shoes.
• Current team census approximately 200 patients.
• EMR: EPIC.
• Support: nursing supervisors, managers and team leads available.
