Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC required or must be obtainable prior to start.
• Current experience in skilled nursing facility or long-term care required; experience with short-term rehab preferred.
• Experience managing caseloads that include long-term care, short-term rehabilitation, and assisted living residents preferred.
• Ability to provide therapy in a small SNF environment with approximately 34 certified beds.
• Minimum of one year of clinical experience as a licensed Physical Therapist preferred.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Guaranteed Hours: 35 hours per week.
• Schedule: Flexible schedule; primarily Monday through Friday with options for 4 or 5 days per week. Weekend coverage may occasionally replace weekdays.
• Start Window: Beginning of May start date, flexible.
• Assignment Duration: Flexible.
